Transforming Case Phase 1

The Transforming Care programme was designed to support the delivery and maintenance of 18 week wait, in conjunction with the South Central backlog initiative.  Working with the alliance of ATOS/Unipart, 24 pathways were redesigned using LEAN methodology while skills were transferred to NHS staff to support the implementation and sustainability of the pathways.  Most PCTs carried out three pathway redesigns although some carried out two.

We are the only SHA to have delivered a holistic approach to addressing the 18 week issue, we have, uniquely, used Lean across care settings (rather than within one organisation) to support a positive patient experience and we have focused on delivering a sustainable system in terms of high quality pathways and residual skills in NHS organisations to continue this important and exciting work.

Objectives

Transforming Care is designed to support the delivery and maintenance of 18 week wait, in conjunction with the South Central backlog initiative.

Build the capability within organisations to manage demand and capacity efficiently and effectively, to ensure the best patient care is delivered in the most appropriate setting and location at an affordable price:

  • Define and implement new pathways for delivering efficient, safe, high quality patient care, removing waste and ensuring all activities are high quality and patient-focused
  • Create Service Improvement capability in South Central through the transfer and development of skills in pathway redesign, process improvement and uality management, project and programme management
  • Build leadership skills in those holding executive positions in organisations cross the SHA

Transforming Care Phase 2

The Board of Commissioners originally signed up to Phase I, building on the success of Phase I, to support the delivery of PCT Operational Plans and to support change process within local provider organisations.  The SHA went out to tender for the programme on this basis and framework arrangements were put in place with:

  1. A Consortium of Atos Consulting and Unipart,
  2. A Consortium of Amtech, Saigei, AMNIS and KM&T,
  3. Tribal Consulting

The original agreement was put in place until 31st March 2009, at the last BoC meeting agreement was gained to extend this for a further year until 31st March 2010.  The agreement can be accessed to help support implementation of LEAN projects undertaken by any organisation within South Central.  The three approved suppliers on the framework can be approached to providework and support in any or all of the following areas:

  • Pathway redesign and implementation
  • Policy deployment
  • Organisational design and development
  • Build, Share, Procure pathway development
  • Knowledge Management, using Map of Medicine
  • Change Management and Organisational Readiness support
  •  Operational redesign and implementation support
  • Programme or project management  

Individual organisations can call off any of these services from the agreement that will help in delivering your Operational Plans or local change programmes.  Developing the framework has enabled us to get excellent value for money.
