NHS North Yorkshire & York (NHS NYY) covers a population base of 794,532; with over 6,000 severe COPD and Heart Failure patients. NHS NYY’s strategy for long-term conditions includes one of the largest telehealth programmes in the UK. 2,000 patients who are currently living with long-term, chronic conditions will be provided with telehealth systems as part of a transformational programme which includes the redesign of services and pathways. Patients will be more appropriately supported so that they can remain in their own home and have a greater level of confidence and awareness about their condition. Supporting patients in this way will significantly reduce the volume and frequency of unplanned hospital admissions which will in turn reduce costs in primary and secondary care.

Milton Keynes Council and Milton Keynes Community Health Services (PCT) are working together to develop effective ways of helping people in the community who are living with a chronic and unstable long-term condition. The aim is to reduce unnecessary admissions to hospital and to facilitate early supported discharge from hospital following an exacerbation of chronic obstructive pulmonary disease (COPD). In order to achieve this aim, telehealth monitors from Tunstall are provided to patients with COPD, to support a more proactive and preventative model of care.
